Medical Guidelines for Sports Participation

Every student is required to have an annual physical prior to tryouts. This physical must be documented on the new State of Connecticut Department of Education Health Assessment Record. This form is available on-line and in the nurse's office.

The physical needs to be completed after June 15, 2010 and returned to Sacred Heart Academy before August 15, 2010. The school nurse will review and clear students for tryouts in August. Each physical is valid for 13 months.

We recommend retaining a copy for your records prior to submitting the form to Sacred Heart Academy.

**Please note: We are transitioning from our two-page Sports Participation Health form to the new State of Connecticut Department of Education Health Assessment Record during the 2010-2011 school year. Our previous sports form will be accepted until June 2011, after which the new form will be the only form accepted.
